One of the biggest challenges our communities face is ensuring organizations have the right resources to drive significant and positive change to the health and well-being of those that need it most. Positive change starts with Social Determinants of Health


Social Determinants of Health (SDoH) are the conditions in the places where people live, learn, work and play that affect a wide range of health risks and outcomes. These circumstances are shaped by the distribution of resources at national, state and local levels. SDoH are mostly responsible for the unfair differences in health status seen within our communities. By continually monitoring conditions such as employment opportunities, public health programs and access, education opportunities, housing stability, etc., communities can better focus policies and resources to improve health at the individual and population level, and advance health inequity.
