Community CareLink’s referral tool allows your organization to refer individuals to other agencies for additional social needs, group meetings, assessments, treatment, specialized counseling, employment services and more. The referral tool is integrated into the patient record, so you can make appointments within the platform, track where you referred an individual, if they attended, and the progress made at the visit. Track that these needs are not only identified, but then met means less documentation for you, more holistic treatment for the community.

By being collaborative with community partners, agencies are able to enhance the care they give by providing more complete services and access to programs and resources their populations need.
