Collaboration for Better Care: Community CareLink’s Referral Tool

Collaborative, integrated referrals

Community CareLink’s referral tool allows your organization to refer individuals to other agencies for additional social needs, group meetings, assessments, treatment, specialized counseling, employment services and more. The referral tool is integrated into the patient record, so you can make appointments within the platform, track where you referred an individual, if they attended, and the progress made at the visit. Track that these needs are not only identified, but then met means less documentation for you, more holistic treatment for the community.

By being collaborative with community partners, agencies are able to enhance the care they give by providing more complete services and access to programs and resources their populations need.


Not only does Community CareLink’s referral tool help meet specific needs of an individual, but by tracking referral data, we have a better picture of a community's specific needs and the services people are seeking that they can’t find. An organization can look back retrospectively at their referral data and adjust programming, services or intake processes based on this new knowledge of the need. We can see where individuals are coming from and where they are going. Our vision as we get more and better data is that we can drive behaviors.
